Critical Notarization Requirements
Forms marked with 🔴 “REQUIRES NOTARIZATION” must be notarized by a licensed notary public before Five Star Towing can accept them. We cannot:
- Accept non-notarized forms that require notarization
- Release vehicles without proper notarized authorization
- Make exceptions under any circumstances
Where to Get Forms Notarized
Most banks, credit unions, UPS stores, shipping centers, law offices, county clerks, and mobile notary services offer notarization. Tip: Bring a valid government-issued photo ID when visiting a notary. Many locations provide notary services for free to customers.
How to Complete & Submit Forms

- Print the form on standard 8.5″ × 11″ paper.
- Complete all required fields legibly and thoroughly.
- Get forms notarized if they are marked 🔴 REQUIRES NOTARIZATION.
- Bring completed forms and all supporting documentation to our facility.

NOTARIZATION REMINDER: Forms requiring notarization MUST have a notary public’s signature, stamp, and seal. Forms without proper notarization will be rejected. Most banks offer free notary services to customers.
Vehicle Release & Impound Forms
This form is mandatory if anyone other than the registered owner is picking up the vehicle. It provides legal authorization for third-party pickup and is required under Oklahoma state law. Without notarization, we cannot release the vehicle to a third party.

Used when a vehicle owner authorizes their insurance company to retrieve the vehicle directly from our facility. Signing this form also confirms that the insurance company assumes all associated fees. Proper notarization is required for the release to be valid.

An official Oklahoma Tax Commission form allowing the release of a vehicle without current insurance. By completing this affidavit, the vehicle owner certifies that the vehicle will not be driven on public roads. This is an important legal safeguard and must be notarized.

A comprehensive checklist of all acceptable documentation required to release a vehicle, including:
- Proof of ownership
- Valid identification
- Insurance verification (if applicable)
This form ensures compliance with Oklahoma law and helps our staff process your vehicle release quickly and efficiently.
Used when immediate family members or legal representatives need to retrieve a vehicle because the registered owner has passed away or become legally incapacitated. Proper notarization is required to confirm authority.

Allows vehicle owners to retrieve personal belongings from their impounded vehicle without paying full vehicle release fees. This service is available free of charge during business hours.
Grants Five Star Towing (or a designated agent) the legal authority to handle vehicle registration, title transfers, and licensing matters on behalf of the owner. Must be notarized to be valid.

An official Oklahoma Department of Public Safety form (DPS 180WS 009) that allows property owners to legally request the removal and impoundment of abandoned or unauthorized vehicles from private property.
